How do I submit a Medical Withdrawal?

Once you complete Forms A, B, and C, please return them to the College of Graduate Studies (Trevor Colbourn Hall, Rm 205A). You can also email these forms to Graduate Student Services (gradservices@ucf.edu). Your medical provider should complete Form D, which should then be mailed to our office directly from the medical provider.
